Epoetin Alfa (Erythropoietin)
Epoetin Alfa (Erythropoietin) is used in veterinary medicine for nonregenerative anemia, especially renal anemia.
Group
Erythropoiesis-stimulating agent
Mechanism of Action
Recombinant erythropoietin stimulates erythroid production.
Dose Rates
Dog
| Use / stage | Dose | Route | Schedule | Instructions |
|---|---|---|---|---|
| Renal anemia induction | Reference only | SC | 35 or 50 IU/kg three times weekly, adjusted to hematocrit; source upper total 400 IU/kg per week. Weekly cap and calendar schedule require dedicated protocol support. |
Cat
| Use / stage | Dose | Route | Schedule | Instructions |
|---|---|---|---|---|
| Renal anemia induction | Reference only | SC | 100 IU/kg three times weekly until target PCV, then reduce to twice weekly and later once weekly. | |
| Renal anemia maintenance | Reference only | SC | 75–100 IU/kg twice weekly, or adjusted to once weekly after stabilization; do not convert twice weekly into an assumed fixed-hour schedule. |
Ferret
| Use / stage | Dose | Route | Schedule | Instructions |
|---|---|---|---|---|
| Anemia treatment | Reference only | IM | 50–150 IU/kg three times weekly; reduce to once weekly if red-cell indices improve sufficiently. Calendar-based induction is not an exact fixed-hour interval. |
Rabbit
| Use / stage | Dose | Route | Schedule | Instructions |
|---|---|---|---|---|
| Anemia treatment — Initial | 50–150 mg/kg | SC | Every 48–72 h | Continue until PCV normalizes, then use the maintenance course. |
| Anemia treatment — Maintenance | 50–150 mg/kg | SC | Every 168 h | Continue for at least four weeks after PCV normalization. |
Indications
- Nonregenerative anemia, especially renal anemia
Contraindications
- Stop with suspected antibody-mediated aplasia or systemic hypersensitivity.
Adverse effects
- Pure red cell aplasia from cross-reacting antibodies
- Seizures and injection reactions

Monitoring and Administration
Monitor hematocrit, reticulocytes, iron status and blood pressure. Typical target PCV is approximately 30–34%, with feline protocols sometimes targeting 30–40%.
Ensure iron adequacy. Darbepoetin is often preferred for prolonged treatment because of the antibody risk.
Store Epogen at 2–8°C, protected from light; do not freeze or shake. Discard single-use vial remainder.
